Yes, you need to know the internals of your mail storage. Actually I do 
something like this from the command line with Maildir sometimes.
Maybe you cause your client to redownload all messages again, maybe 
duplicate them or make messages inaccessable.

Maybe, it depends on the storage you are using. With mbox and Maildir 
Dovecot should detect the change and re-index itself.
