To me this looks like io_remove() doesn't work properly in the kqueue
code. notify_callback is NULL only after the notification is removed,
and index_mailbox_check_remove_all() has been called.

BTW. Which IMAP client does this? Someone could tell them not to send
tag with DONE command..
