[Dovecot] failed to create storage data

Are you returning "mail" or "home" from sql? You probably want it to 
return "home", and have default_mail_env as:

default_mail_env = maildir:%h

Also earlier Dovecot versions didn't try to create directories for the 
new accounts, so maybe the problem is just that?
