I guess so

Well it applies cleanly (after substituting "exim-4.63" for "exim-4.43")
and seems to build OK (on Solaris 10)

No, but I'd try what was suggested in the "authenticators" section;
something like :-

begin authenticators

ntlm:
        driver = dovecot
        public_name = NTLM
        server_socket = /var/authentication_socket_path
        server_set_id = $1

... etc.
